All patients must stop by the Admissions Department in the main lobby to complete the registration process.

PLEASE NOTE: YOUR  PRE-REGISTRATION  MUST  BE  COMPLETED  AT  LEAST  24  HOURS  PRIOR  TO  YOUR  VISIT.

Wayne Memorial Hospital’s online patient pre-registration process is a handy, time-saving option that allows you to pre-register at your convenience. We recognize the confidential nature of the information you submit online. For this reason, you will be transferred to a secure page to complete the pre-registration process.

Before you begin the pre-registration process, you will need the following:

• Insurance ID card information
• Procedure Date & Time
• Type of Procedure (e.g. mammogram, xray,lab,etc.)
• Personal Contact Info
• Physician Order
